One of your staff members approaches you about an upcoming conference and asks to attend. You decide it’s a good idea and give the go-ahead. But when he goes to fill out a formal request for time away from work, you both discover that you’re out of forms. You ask another floor to share some forms, and they, too have to dig them out. They finally send one up, your staffer fills it out and you sign your o.k. Then it’s off to interoffice mail for the Human Resources stamp of approval. But two days later, when HR calls back, it’s not with an approval, but to inform you that you and your staff person filled out an older version of the form and must now re-submit your request. FormFastSite is a web-based solution offering storage and processing of forms online. Users can view forms, fill them out on line and mail them, or print/download forms for later use.

Here’s how it works: A hospital which subscribes to FormFastSite is provided with a website or website link which contains all the forms. Authorized users may access the website using a password, which is issued by the hospital’s FormFastSite Administrator.

Users may include employees, physicians, patients or prospective patients. The password determines which forms the user may access on the site. Once there, the user may want to view a particular form, such as a policy or procedure. Or, to offer another example: Someone who is pre-registering for an outpatient procedure might choose to access instructions, or directions to your facility.

With FormFastSite, users may also download and print blank forms. This makes FormFastSite a central distribution source for all authorized users. Not only does it eliminate the need to physically distribute printer forms to users, it also provides a way to update forms online at a central source.

And if stacks of paper supplies begin to look like the forests they came from, forms can also be completed online using FormFastSite. They can then be sent via email to other departments for additional processing (Remember the request for time-off to attend a conference?).
